SYMPTOMS
========
During authentication, if the account is locked out and the Internet Information
Server (IIS) computer is logging to an ODBC database, the following error
message occurs:
Overlapped I/O error
This error message is incorrect. Users should receive an error message stating
that their user accounts have been locked out.
CAUSE
=====
The authentication error that the account has been locked out is being reset by
a pending I/O error returned from an asynchronous write to the IIS log. This
causes the incorrect error message to occur.
RESOLUTION
==========
Windows NT Server or Workstation 4.0

To resolve this problem, obtain the latest service pack for Windows NT 4.0 or the

Microsoft has confirmed this to be a problem in Windows NT 4.0 and Windows NT
Server 4.0, Terminal Server Edition.
This problem was first corrected in Windows NT Server 4.0 Service Pack 6 and
Windows NT Server 4.0, Terminal Server Edition Service Pack 6.
