Netfinity 7000 / PC Server 704 - Fore Systems ATM adapter PCA-200EPC, Win NT 4.0, Using multiple adapters

Problem:
When multiple (two, three, or four) adapters are installed in a PC Server 704 or Netfinity 7000, one or more of the adapters are not found (recognized). 
Solution:
The problem is related to the PCI slot scanning order by the adapter driver, versus the BIOS's PCI slot scan order.  The FORE ATM adapter driver scans the cards in reverse order so the user must attach the ELAN driver (forelan.sys) to the hardware ATM driver (foreatm.sys) in reverse order.  From the NT4.0 control panel --> Network --> Adapters setup, the user needs to reverse the association of the ELAN driver with the Adapter number as follows:
 for 2 adapters;  ELAN 1 to adapter 2, and ELAN 2 to adapter 1
 for 3 adapters;  ELAN 1 to adapter 3, and ELAN 3 to adapter 1
 for 4 adapters;  ELAN 1 to adapter 4, and ELAN 4 to adapter 1, and
                                ELAN 2 to adapter 3, and ELAN 3 to adapter 2
NOTE:  This assumes the physical cabling connects the ATM switch for ELAN 1 to the Fore adapter physically located in the first PCI slot (defined as the first slot scanned by the machine's BIOS).  The cable from the ATM switch for ELAN 2 connects to Fore adapter located in the second PCI slot (scanned by BIOS), etc. and the last ELAN ATM switch cable is connected to the Fore adapter in the PCI slot scanned last by the BIOS.
