IBM IntelliStation / IBM PC / Netfinity 3000 - System hang with Auto 16/4 LanStreamer PCI Adapter

Symptom
After installing an IBM Auto LANStreamer 16/4 PCI Adapter, P/N 04H8095, alone, or in combination with one or more additional PCI adapters (SCSI, Ethernet, etc.), the system hangs upon pressing Ctrl-Alt-Del to log onto Windows NT.
Problem Isolation Aids
This tip applies to the following IBM machine types preloaded with, or running Windows NT 4.0.
IBM PC300PL Series 6862/6892
IBM PC300GL Series 6275/6285
IBM IntelliStation E Pro Series 6893
IBM Netfinity 3000 Series 8476
The hang condition is seen when the Auto LANStreamer 16/4 PCI Adapter is installed in the bottom PCI slot on the riser card alone, or in combination with other PCI adapters.
Fix
The IBM Auto LANStreamer 16/4 PCI Adapter will not function correctly in the bottom PCI adapter slot which shares IRQ 9.
The IBM Auto LANStreamer 16/4 PCI Adapter must be moved to either the top, or middle PCI adapter slots on the riser card.
