IBM IntelliStation M Pro (Type 6889, 6898) - Intermittent video loss/corruption with Permedia 2/2A

SYMPTOM:
The display exhibits intermittent video loss, or video corruption (distortion and/or vertical lines on the display).
The display may also exhibit jitter or brief flashes when operating at high resolutions and refresh rates (1024x768xTrue Color @ 100Hz).
PROBLEM ISOLATION AIDS:
This tip applies to the following IBM IntelliStation M Pro Series systems:
IntelliStation M Pro 6898 with Permedia 2 Adapters
IntelliStation M Pro 6889 with Permedia 2A Adapters
FIX:
Before replacing hardware, ensure the Permedia 2/2A video adapter is fully seated and making adequate contact by reseating the adapter 3 to 4 times. Restart the system and test for the video corruption condition.
If the condition is persistent or reoccurs, replace the Permedia 2/2A adapter.
