IBM PS/2 - SCSI adapter in 8550,8555 model 031

Symptom:
Certain PS/2 SCSI adapters may fail when installed in an 8550 or 8555 configured with a 30MB (ST506 interface) fixed disk drive, and an attached external SCSI fixed disk drive. Affected SCSI adapters fail at power at 'Power On' and when using DOS, a 'Bad or missing command interpreter' message appears. A similar appropriate message appears when using other Operating Systems.
Problem Isolation Aids:
Affected SCSI adapters are marked with 64F1356 located in the top right corner on the component side of the card.
Fix:
Should the above conditions exist, replace the PS/2 SCSI adapter, FRU P/N15F6561.
