16-bit AT Fast SCSI Adapter - SCSI devices not recognized on the SCSI adapter

Symptom:
- SCSI devices are not seen when attached to the IBM 16-bit AT Fast SCSI Adapter.
- The adapter BIOS fails to load.
Problem Isolation Aids:
- The adapter is an IBM 16-bit AT Fast SCSI adapter (Plug and Play). The Marketing part number is 06H9649 and the FRU is 07H0511.
- SCSI devices connected to the adapter are not seen.
Fix:
1. Power off system, re-seat adapter, reseat all existing SCSI cable connections.
2. If step 1 does not resolve the problem, replace the SCSI cable with SCSI cable FRU p/n71G2558. Make certain that an internal SCSI device can be jumpered to provide termination or install the In-line Active Terminator FRU p/n61G2448.
Note: The original SCSI cable that shipped with the AT adapter was manufactured with a "built-in" terminator which cannot be removed. FRU p/n71G2558 has a 3 year warranty through customer warranty claims. FRU p/n61G2448 has a 1 year warranty through customer warranty claims.
