Ordering by cash, credit, check, or money order:

To order and register full use by mail, send this order form 
and payment (plus Florida sales tax if applicable) to:

Payment must be in US dollars drawn on a U.S. bank, or you may 
send international postal money orders in U.S. dollars.

Credit Card Ordering Information:

To order by credit card, you may e-mail this form (filled in) to 
MatterWare@aol.com. You may also mail credit card orders to the 
postal address above. Please be sure to indicate your credit card
type (Visa, MasterCard, or AmEx), number, and expiration date 
on all credit card orders.

Credit card, cash, and money order orders will be shipped within 
10 days of receipt of order. Orders payed by check may take as
many as additional 10 days.

Family Matters regular price is $30 plus applicable tax. 
It will be available for the introductory price of $20 
(plus applicable sales tax) through 31 Dec 1995. If your address
is in the state of Florida, you must pay sales tax on the 
order amount. If the mailing address is within Florida, please
also indicate your county of residence. Please add $5 shipping 
and handling for deliveries outside the U.S.A. for each copy.
