 AB    OBJECTIVE: To provide basic medical care, counselling and home-based
       service to ease the economic burden of hospitalization of AIDS patients.
       METHODS: 42 AIDS patients diagnoses at Connaught Hospital in 1993 were
       sent to their various homes on request for a follow-up counselling and
       medical care services. The effectiveness of home-based care in terms of
       services delivered and the average cost involved per patient were
       determined. RESULTS: 80% of the relatives of AIDS patients admitted with
       satisfaction the services rendered to the patients at home. The
       incidence of stigmatization and the violation of confidentiality were
       reduced by 60% and 70% respectively. The cost for home visit per patient
       compared to hospitalization droped by 80%. CONCLUSION: Home-based care
       offers better services than the already over-burdened and crowded
       hospitals in Sierra Leone. These services need to be extended to the
       community.
