 AB    OBJECTIVE: 36% of AIDS in Geneva are intravenous drug users (IDUs).
       Preventive strategies: syringe exchange, methadone, free condoms. How
       does the HIV-epidemic evolve in IDUs since institution of these
       measures? METHODS: Data from: 1) Geneva part of the Swiss HIV Cohort
       Study, with 6-monthly follow-ups 2) Geneva Methadone Cohort Study, with
       more than 500 IDU enrolled since February 1988 and tested for HIV every
       6 months. RESULTS: Comparing 1993 and 1991: decrease in newly discovered
       cases of HIV infection (-61%), of AIDS cases (-49%), death from AIDS
       (-26%). Increase in average age at start of follow up from 28.9 years in
       1988 to 31.8 in 1993, decrease in average CD4 count at start of follow
       up (suggesting that newly discovered infections occurred a long time
       ago). Seroprevalence of 8% among those starting methadone treatment in
       1992-93 versus 51% in 1984-85. Seroconversion of 0.95% per person year
       of follow up from 1988 to 1993, compared to > 10% between 1981 and 1983.
       CONCLUSIONS: Epidemiologic indicators of HIV infection consistently show
       a slow-down of the HIV epidemic among IDU in Geneva. It is important to
       continue and adjust existing harm-reduction strategies.
