 AB    OBJECTIVE: To describe trends in AIDS incidence among birth cohorts of
       men reporting sex with men in California county regions. METHODS: AIDS
       cases reported in California by 1/7/94 among men reporting sex with men
       were analyzed, excluding cases meeting the 1993 expanded AIDS case
       definition. Trends in AIDS annual AIDS incidence through 1991 were
       examined by county regions and birth cohorts. RESULTS: In San Francisco
       and Los Angeles counties, AIDS incidence was decreasing by 1991 among
       gay men born before 1945, and increasing substantially among gay men
       born in or after 1960. Between 1987 and 1991, incidence among men born
       1960-64 increased 216% in Los Angeles and 206% in San Francisco, with
       increases from 1987 to 1991 2-3 times higher among those born 1965 or
       later. Cases born 1960 or later (aged 20 or less in 1980) comprised 16%
       of San Francisco cases diagnosed in 1991, and higher proportions
       (21%-26%) in other county regions. Outside the two original epicenters,
       similar trends were observed, with decreases or plateaus in incidence
       among cases born before 1950, and increases among men born thereafter.
       In all regions, incidence tended to increase more rapidly among the
       younger birth cohorts. CONCLUSIONS: AIDS incidence among gay men in
       California is decreasing among men aged 35 or more in 1980, and
       increasing rapidly among those aged 20 or less in 1980. These younger
       men were most likely infected during their twenties. Continued HIV
       prevention efforts targeted at men reporting sex with men are still
       needed, focusing on teenagers and young adults.
