 AB    OBJECTIVE: Contribute to the improvement of the sensitivity of AIDS case
       definition in children to decrease undernotifying METHODS: A
       retrospective study of 299 (34%) of the 875 cases of AIDS notified in
       children under 15 in the State of Sao Paulo since 1984 was performed. We
       registered all clinical manifestations (CM) and opportunistic infections
       (OI) according to age groups: (1; 1-4; 5-9; 10-14 years. RESULTS: The
       most frequent CM were fever (44%), weight loss (40%), diarrhea (39%),
       hepatomegaly (37%) and growth impairment (34%). The most frequent OI
       were oral candidiasis (53.5%), bacterial pneumonia (33%), Pneumocystis
       carinii pneumonia (22.7%), tuberculosis (18.5%), and otitis (11.5%). CM
       were evenly distributed among the age groups studied. The frequency of
       OI differed however according to age, as children in the 10-14 age group
       presented OI more similar to AIDS in adults. CONCLUSION: The current CDC
       definition of AIDS case in children is not sensitive enough for defining
       cases in Brazil. The description of the most frequent CM and OI shows
       that new criteria for defining case in our environment is necessary.
