 AB    OBJECTIVE. In the Nordic countries during the last decades comprehensive
       data about drug abuse are available. Since 1984 mandatory registration
       of new HIV infected cases is performed. The combination enables
       valuation of the present spread of HIV. METHODS. These unique statistics
       in the authorities responsible for drug abuse and epidemiological survey
       are compiled. RESULTS. The number of IVDU is 5-10.000 in each of
       Denmark, Norway and Sweden but much fewer in Finland and Iceland. Heroin
       prevails but amphetamine is widely used as the drug injected. In Norway
       and Sweden 80-96% of IVDU are HIV antibody tested and here also the
       development of AIDS and HIV is similar. HIV was introduced among IVDU in
       1983 and the peak spread took place in 1984-85. After 1987 there was a
       steep fall in incidence, thereafter levelling out. The HIV prevalence in
       the capitals is around 10%. Perinatal transmission from IVDU mothers has
       ceased. CONCLUSIONS. The main factors limiting the spread of HIV among
       IVDU probably were early extensive information, testing, counselling,
       individual knowledge of HIV status and adequate care.
