AB OBJECTIVE: To ascertain the time trends of HIV infection in population
groups with different risk factors in the area of Madrid. POPULATION
ANALYSED: We have studied 6.691 subjects (5.804 men and 887 women) with
a mean age of 29.6 years (range 17-70), who first attended in a STD/HIV
regional clinic between 1.1.88 and 1.1.94, with the aim of ascertaining
their HIV serological status. In accordance with HIV infection risk
factors, these 6,691 subjects were grouped as follows: 2985 injecting
drug users (2.098 males and 887 females) and 3706 homo/bisexual men.
METHODS: A HIV serology was performed of all the patients (ELISA
1st-2nd/3rd generation and confirmation by Western-Blot). RESULTS:
Overall HIV seroprevalence was 35.0% (2.342/6.691). The annual
prevalence of HIV infection in the groups studied is shown in the table
below: TABULAR DATA, SEE ABSTRACT VOLUME. We have observed a gradual
increase in the demand for the anti-HIV antibody detection test in
homo/bisexual men and a contrary trend in injecting drug users. We found
the following trends in the annual prevalence of HIV infection:
       1.--clearly downward, after 1990, in homo/bisexual men; 2.--Slightly
downward, after 1991, in I.D.U.. CONCLUSION: Knowledge of the
dissemination of HIV infection, obtained in suitably representative
seroepidemiological studies, would allow a preventive health policy with
adequate allocation of technical and human resources.
