 AB    OBJECTIVE: To describe pentamidine pharmacokinetics in a woman with AIDS
       and her newborn infant while the mother was receiving treatment for
       Pneumocystis carinii pneumonia (PCP). METHODS: (Case Report): A
       21-year-old HIV seropositive female who was 32 weeks pregnant was
       hospitalized with PCP (CD4 count = 196). TMP-SMZ was stopped due to
       adverse events and she was placed on i.v. pentamidine 200mg each day
       (weight = 58.1 kg). Due to progressive worsening of the mother's medical
       status, the neonate was delivered by C-section. Blood specimens for
       pentamidine concentration were sent that day from the mother and neonate
       (cord blood). RESULTS: The mother's pentamidine concentration 8 hours
       after her 7th dose was 81.3 mcg/L (a simulation model predicted 3-76
       mcg/L). The infant's concentration drawn 16.5 hours after the mother's
       7th dose was 13.2 mcg/L (Table 1). CONCLUSION: Present knowledge of the
       pharmacokinetics of placental transfer of antimicrobial agents used in
       the treatment of HIV/AIDS related infections is very limited. Data
       regarding placental transfer of pentamidine is limited to data from in
       vitro or animal models. In this report, in vivo placental transfer of
       pentamidine was observed. Networks to further study maternal-fetal
       pharmacokinetics of medications used in the treatment and prophylaxis of
       HIV-related complications need to be established.
