AB OBJECTIVE: To determine the detectability of IgM antibodies to HIV-I gag
and env protein during various stages of HCV infection by using a very
sensitive ELISA method. METHODS: Twenty commercially available
seroconversion series and 200 asymptomatic carrier samples were tested.
A very sensitive HIV IgM ELISA was established using microELISA plates
       coated with recombinant HIV-1 p24, purified viral gp160 and HIV-2 gp36
peptide (aa 592-603). Data were compared with those of second generation
(anti-IgG) and third generation sandwich type HIV screening tests.
RESULTS: All twenty seroconversion series were IgM positive, in some
cases more than two weeks before IgG becomes detectable. Third
generation sandwich type HIV screening assays (c.q. Vironostika HIV
Uni-Form II) are as sensitive to early seroconversion as the IgM
specific test. Surprisingly some 50% of asymptomatic carrier samples
contained detectable amounts of IgM as well. CONCLUSION: Anti-HIV IgM is
a consistent marker of early seroconversion and early detection of
seroconversion, therefore requires a good sensitivity to IgM-type
anti-HIV. Anti-HIV-IgM however, may persist during the asymptomatic
stage of HIV infections. Future studies are aimed at a potential
predictive value of this marker for disease progression.
