AB OBJECTIVES: To compare the accuracy of a saliva test protocol in
determining the HIV-1 antibody status of individuals in various clinical
settings and stages of HIV disease. METHODS: Paired samples of serum and
saliva were obtained from 1256 subjects at four study sites in
Vancouver: 337 from a hospital-based infectious diseases clinic serving
HIV seropositive patients, 478 from an AIDS testing clinic, 10 from an
outpatient hemophilia clinic and 431 from a street-based nurse outreach
program. Serum was tested using standard methods. Saliva was tested
using Recombigen HIV-1 EIA (Cambridge Biotech). Reactive saliva EIA
tests were retested singly and further tested by
radioimmunoprecipitation (RIPA). RESULTS: There were 368 seropositive
and 888 seronegative subjects. HIV antibodies were detected in saliva as
follows: TABULAR DATA, SEE ABSTRACT VOLUME. HIV antibodies were not
detected in saliva from seronegative individuals (specificity = 100%).
DISCUSSION AND CONCLUSIONS: This study demonstrated good agreement
between the results obtained from saliva and serum testing for HIV
antibodies. Because the sensitivity of saliva testing is lower than for
serum, it cannot be recommended for diagnostic testing but should prove
to be a useful tool for epidemiologic purposes.
