 AB    OBJECTIVE: To compare saliva and serum samples for HIV testing. METHODS:
       Duplicate samples of saliva and serum were collected from 286 subjects
       at high-risk of HIV infection in three different settings in Barcelona.
       HIV testing was done through commercial ELISAs in both samples following
       WHO strategy II. Discrepant results between serum and saliva were
       further tested with Western blot. RESULTS: The provision of a saliva
       sample using a commercial saliva collector was accepted by the
       participants without any particular problem, and agreement between
       antibody HIV testing using saliva and serum specimens was 98.9% (95%
       confidence intervals: 97.7-100). DISCUSSION: If blood samples are not
       available saliva is an effective alternative to serum for HIV surveys
       among high risk individuals. Furthermore, a three-step protocol similar
       to the one we used is an acceptable strategy for epidemiological studies
       were data has to be analyzed at the individual level, especially if the
       Western blot or other confirmatory tests are further optimized for use
       with saliva.
