AB OBJECTIVE: The aim of the present study was to examine whether the
general HIV-related decline in CD4+ T-lymphocytes and CD4+/CD8+ ratio is
reflected in gingival tissues, and thus can constitute a possible
explanation of HIV-infected persons susceptibility to periodontal
diseases. METHODS: Frozen sections of gingival biopsies taken from 31
HIV-infected persons (11 with gingivitis and 20 with periodontitis) and
14 HIV seronegative persons with periodontitis were stained with
monoclonal antibodies against CD4 and CD8. Positive cells were counted
and CD4/CD8 ratio compared with the same ratio in peripheral blood.
RESULTS: In the HIV-infected persons consistently fewer CD4+ than CD8+
cells were found in the gingival tissues, whereas in the HIV
seronegative group fewer CD8+ than CD4+ lymphocytes were common. The
CD4/CD8 ratio was significantly higher in the gingiva than in peripheral
blood in the HIV-infected group, but the ratio was the same in the two
localizations in the HIV seronegative group. CONCLUSIONS: HIV-related
immunodeficiency is reflected in gingiva as a low CD4/CD8 ratio. This
might lead to raised susceptibility to periodontal diseases. The higher
CD4/CD8 ratio in the gingiva compared to peripheral blood means that
CD4+ cells are attracted to inflamed areas also when these cells are
       few.
