 AB    OBJECTIVE: (1) To compare incidence of oral lesions among HIV-1 infected
       persons by route of transmission of HIV (sexual or parenteral). (2) To
       estimate the effect of oral lesions by group on the incidence of AIDS.
       METHODS: We examined 76 HIV-1 infected homosexual men and 72 men and 24
       women infected through contaminated blood products (59% hemophiliacs,
       41% transfusion recipients; TRs). Each underwent semi-annual oral exams
       (for candidiasis and hairy leukoplakia), H&P, and measurement of CD4
       counts, from soon after infection till Dec 1993 or loss to follow-up. We
       estimated incidence using proportional hazards and Kaplan-Meier methods,
       adjusted for missed exams and changing CD4 counts. RESULTS: At baseline
       (July 1987), 89% of subjects had CD4 counts > 200 cells/microliters. All
       homosexuals were 18-64 years of age, while 78% of hemophiliacs and 62%
       of TRs were in this age group. Parenterally-infected subjects developed
       oral candidiasis sooner than homosexual men (p = .04), but developed
       hairy leukoplakia slightly later than these subjects (p > .20). Relative
       hazards for AIDS due to oral lesions will also be reported. CONCLUSIONS:
       These findings indicate that the risks of specific oral lesions depend
       on route of HIV-1 transmission, having implications for management of
       HIV disease in women and children as well.
