AB OBJECTIVE: To identify temporal trends of AIDS associated Kaposi's
sarcoma (KS) in Spain. METHODS: Data from 18.256 AIDS cases reported to
the Spanish AIDS Registry from 1981 to 1992 and data from a
retrospective cohort of AIDS patients from three different Autonomous
Communities (Madrid, Baleares and Catalonia) were used. Follow-up data
included both clinical outcome and death. A total of 3.945 AIDS patients
followed for 6.296 person years were included in the cohort. RESULTS:
Starting in 1985 and for each subsequent year the KS presenting
       percentage was as follows: 14.4, 11.04, 9.28, 6.4, 6.2, 6.0, 6.3 and 6.2
in 1992. No significant trends were identified after the introduction of
       the 1987 CDC AIDS definition criteria (p = 0.43, 0.35 and 0.29 for all
cases, IVDUs and homosexual men respectively). Annual incidence rates by
cohort of diagnosis showed a slight but non significant decrease in the
first year, but a slight and non significant increase in the second year
after AIDS diagnosis. DISCUSSION: In our setting, although KS has not
decreased as an AIDS presenting form, it tends to increase as a
subsequent AIDS diagnosis. Natural history of HIV have changed, and the
assessment of KS trends over time should take in account follow-up data
and specific local features like the prevalence of other opportunistic
       infections, as tuberculosis.
