AB OBJECTIVE: To assess KS epidemiology among patients treated in Harris
County Hospital District. METHODS: A retrospective review of demographic
and clinical variables, among all AIDS patients with KS, treated between
1987 and 1994, identified by coding and surveillance data. RESULTS: Of
286 possible cases, 225 records were available by 2/25/94 (6-10% of all
pts). Study group included 223 men, was largely white (63%) and
homosexual (81%). Median age at KS diagnosis was 34, at HIV diagnosis 32
(interval median, 14 months). KS was AIDS-defining dx in 44%. Median CD4
count at diagnosis was 32/mm3. KS was disseminated at dx in 43%, limited
in 36%. Recent cases (after 1/91) were more often nonwhite (p = 0.03),
associated with nonsexual HIV transmission (p = 0.02) and had oral
lesions (p 0.05). Best clinical responses were with intralesional
interferon (79% complete or partial responses), vincristine/vinblastine
       (55%), or adriamycin/vinblastine/bleomycin (48%). DISCUSSION AND
CONCLUSIONS: KS occurs in 6-10% of all AIDS patients treated in our
public hospital district, is often AIDS-defining, and presents with CD4
lymphopenia. Its epidemiology has changed with time. Certain
chemotherapy regimens are associated with good responses.
