 AB    BACKGROUND: Yunnan Province is located in south western China. Its
       border with Vietnam, Laos and Myanmar is over 4000 Kms long. In 1989 we
       first detected HIV antibodies in injecting drug users (IDUs) from the
       western border areas of Yunnan. Since then Yunnan has had more
       HIV-infected people than any other province. In China more than 85% of
       HIV positive individuals was reported from Yunnan. Near 1000
       HIV-infected people and 21 AIDS cases have been reported as of 31
       December, 1993. METHOD: The data here rely on aggregating the results of
       two surveillance systems including routine surveillance and sentinel
       surveillance which reflects the HIV epidemic as it has progressed in
       Yunnan Province basicly. RESULTS: The pattern of the HIV infection
       epidemic in Yunnan shows the following characteristics: 1) In the border
       prefectures of western Yunnan, a major epidemic among IDUs has been
       demonstrated. The prevalence of HIV infection is 5-80% in Dehong
       Prefecture; 2) In the border prefectures of south-western Yunnan, a
       major sexual transmission of HIV is occurring, especially among the
       young women returning from Thailand and Myanmar; 3) A mixed pattern of
       HIV infection can be found in Kunming city. The first foreign AIDS case
       was found there; then three HIV infected IDUs were reported among
       prisoners from Dehong and Dali prefecture. Recently HIV infection has
       been detected in people returning from abroad and in local prostitutes.
       The epidemiological pattern of HIV infection is similar to that of the
       initial stage of HIV epidemic in Thailand. We must urgently undertake
       Health promotion and perform other preventive measures to reduce the
       spread of HIV in Yunnan province and all of China.
