 AB    OBJECTIVE: To compare HIV viral RNA and P24 antigen as well as hemoral
       response to the infection among seroconverters (SC) who developed AIDS
       in less than 3 years with SC who did not develop AIDS. Seroprevalents
       (SP) with no CD4 decline in 7 years of follow up were also used as
       control. METHOD: 17 SCs who developed AIDS in 3 years, 42 SCs who were
       AIDS free and 56 SPs with no or minimal CD4 counts were selected from
       4952 gay/bisexual men participating in Multicenter AIDS Cohort Study
       (MACS). HIV-1 P24 antigen and virion RNA as well as neoptrin, B-2
       microglobulin, Interferon X, IgG, IgM, IgA and quantitative HIV-1
       serology were performed in plasma. RESULTS: Levels of P24 antigen was
       significantly higher at first positive visit and visit closest to AIDS
       in SCs with AIDS less than 3 years. (0.02 and 0.004 respectively).
       Similarly titer of HIV RNA was 10(4.2) copies/ml and raised in rapid
       progressor as compared to 10(3.4) in non progressor and remained stable.
       Anti P24 and P17 was different in two groups (P = 0.02 and 0.0002
       respectively). Data on neopterin, B-2, Ig's and Interferon show no or
       narrow correlation. CONCLUSION: The quantitation of viral antigen and
       RNA and anti-HIV antibodies serve as predictors of progression to
       disease stage among gay/bisexual men.
