 AB    OBJECTIVES: a) to identify the possible correlation between clinical
       features of HIV infection and spontaneous lymphocytes apoptosis level
       (SA), b) to verify if abnormal expression of molecules involved in T
       cell costimulation is a feature is a feature of cells undergoing AS.
       METHODS: nuclei of non-stimulated peripheral blood lymphocytes (PBL)
       cultured for 72 h have been stained by propidium iodide and SA level
       measured by cytofluorimetric assay. RESULTS: 96 HIV+ pts and 69 HIV-
       healthy subjects have been prospectively studied. We have observed that
       the rate of apoptotic cells (1) is significantly higher in HIV+ pts than
       in healthy controls (mean + DS: 13.7 + 11.8% vs 3.7 + 1.5% p < 0.001),
       (2) it is higher in AIDS pts than in other HIV+ pts (19.9 + 13.6% vs 7.5
       + 4.3%, p < 0.001), (3) it is weakly. albeit significantly. inversely
       correlated with both CD4+ (r = 0.44. p < 0.001) and CD8+ (r = -0.35. p <
       0.001) counts, (4) Long-term survivors pts (LTS) had a significantly
       lower SA level than asymptomatic HIV+ pts with a shorter follow-up and a
       similar number of CD4+ circulating lymphocytes (5.8 + 2.9% vs 9.9 +
       4.9%. p < 0.05). In LTS. but not in other HIV+. SA levels remained
       stable over time. CD28 expression is significantly lower in CD4+ and
       CD8+ from HIV pts compared to controls (CD4+ p < 0.001: CD8+ p < 0.001).
       DISCUSSION AND CONCLUSION: SA levels may correlate with impaired
       inter-cellular signal transduction (low CD28) and may represent the
       final result of incomplete HIV mediated activation followed by
       reactivation. SA levels may have prognostic significance, even if
       further studies are needed.
