AB OBJECTIVE: To identify significant prognostic factors influencing
survival (SV) in a seroprevalent cohort. METHODS: Between 1/1984 and
5/1992, 328 HIV (+) pts. entered into this cohort. Features at diagnosis
were: median age 27 years (range 16-63) gender: male (M) 256, female (F)
72; risk behavior: homosexuals (HS) 158, intravenous drug abusers (IVDA)
126, heterosexuals (HTS) 44; Stage (St) according WHO Staging System:
1:212, 2:26, 3:63, 4:27. SV distributions were evaluated by the method
of Kaplan-Meier and differences in time distribution were evaluated
using log-rank test; the Cox proportional hazard model was used to
assess the joint effect of prognostic factors on SV. RESULTS: Median
       time to progression (TTP) was: age < or = 27:64 mo, > 27:38 mo (p =
       0.0000); M 44 mo, F 69 mo (p = 0.0010); HS 38 mo, IVDA 75 mo, HTS 60 mo
       (p = 0.0000); St 1 63 mo, St 2 43 mo, St 3 43 mo (p = 0.0000); median
TTP was 50 mo. Progression to AIDS for initial St 1, 2 and 3 was 74/212,
9/26 and 25/63 respectively, 75 of them died. Median SV was: age < or =
       27 not reached (NR), > 27 65 mo (p = 0.0000); M/F: 64/75 mo (p =
       0.0016); HS 64 mo, IVDA NR, HTS 57 mo (p = 0.0057); St 1 NR, St 2 NR, St
       3 63 mo, St 4 28 mo (p = 0.0000); median SV was 75 mo. Several Cox
models were performed using pts. characteristics at diagnosis and
progression to AIDS; the following model was adopted as best related to
the regression coeficient: Age and to develop more than one ADI.
CONCLUSIONS: Adverse parameters influencing our pts. SV according
univariate are: Male, homosexual behavior, St 3 & 4, age > 27. By Cox
multiregression analysis age and more than one ADI remain as adverse
       prognostic factors.
