AB OBJECTIVE: To describe the clinical and laboratory data in patients
surviving more than 5 years after an AIDS diagnosis. METHODS:
Retrospective review of computerized medical record of a cohort of AIDS
patients followed from 1th June 1981 to 31th December 1988 by one unit.
Kaplan Meier curve was used to estimate survival. RESULTS: Of the 610
AIDS patients diagnosed before December 1988, 54 (8.8%) survived more
       than 5 years. 53 were male and 1 female: 47 (87%) were homosexual, 5
       (9%) bisexual, 1 IVDU and 1 heterosexual. Mean age was 37 (20-55); 41
(76%) had Kaposi's Sarcoma (KS) as a single AIDS indicative disease
while 13 (24%) had an opportunistic infection (OI): PCP (2),
       PCP+toxoplasmosis (5), oesophageal candidiasis (2), cryptosporidiosis
       (2) and tuberculosis (5). Mean CD4 cell count by the time of diagnosis
       was 415/mm3 (43-1550), (442 +/- 291 for KS group and 328 +/- 218 in OI
group: p-NS). 10 patients had less than 200 CD4/mm3 (7 KS and 3 OI) and
4 less than 100/mm3. Over others biological data mean values were: beta
       2m: 3.9 mg/l, IgG: 22.4 g/l, IgA: 3.2 g/l. Among the 54 patients 21
(39%) had no new AIDS defining condition, 9 (17%) developped a
progression of KS without OI and 24 developped at least one other
manifestation of AIDS. Median survival is 8.01 years (range 5.1-11) with
a cumulative survival of 46% at 8.1 years. For KS mean survival is 8.77
(CI 7.97-9.61) with a cumultaive survival of 57% at 8.1 years. For OI
the median is 5.85 years (range 5.1-7.6) with a cumultaive survival of
years. For OI the median is 5.85 years (range 5.1-7.6) with a cumultive
survival of 45% at 6 years. CONCLUSION: Patients surviving more than 5
years to an AIDS diagnosis represent 8.8% of our study population. Among
those median survival is 8 years until up to 11 years. 24% had an OI as
AIDS indicative diagnosis and 18% and less than 200 CD4/mm3 at the time
of diagnosis. Further studies are in progress to identify factors
       influencing survival in this population.
