 AB    Chemoradiation for epidermoid carcinoma of the anorectum is well
       established. The medical records of 42 patients diagnosed with anal
       cancer treated under protocol between 1987 and 1991 were reviewed.
       Patients received 5-FU and mitomycin C concurrent with radiation of 30
       Gy over 3 weeks to the pelvis followed by 5-FU. Average lesion size was
       3.8 cm, with 14/42 (35%) having lesions greater than 5 cm. Currently,
       with mean follow-up of 33.4 months (range: 18-61), 36/42 patients (86%)
       are alive without evidence of disease (NED). Ten (24%) patients
       recurred. Seven of nine (78%) were salvaged by abdominoperineal
       resection. One failed distantly. Mean time to local recurrence was 18.4
       months (range: 2-59). Two patients (5%) have died of anal cancer, and 2
       (5%) of intercurrent disease. Local recurrence did not correlate with
       protocol compliance, tumor grade, location, histology, or HIV infection.
       Local recurrence did correlate with size of tumor and stage. Additional
       radiotherapy may be required to control local regional anal cancer in
       lesions greater than 5 cm.
