AB In this paper a model of community based service delivery as influenced
by the role of the Clinical Nurse Consultant HIV/AIDS which has been in
use in Tasmania for the last 2 years will be explored. The main aims of
the position were (1) to provide expert nursing consultancy within the
community setting, (2) collaborate with other health workers in the
hospitals & community with the view to integrating HIV/AIDS care within
mainstream health and (3) to allow clients access to brokerage monies
which allow the purchase of goods & services which might prevent
premature/inappropriate hospitalisation. To support this model a nurse's
special interest group & a volunteer support program managed by
Palliative Care Services was formed. These groups dovetail into the
education and community support groups which exist in the North, North
East & North West of the state. The paper will outline how this model of
integrated care has worked and whilst numbers of clients have been low
the quality of care and treatments has been high. The significance of
HIV in Tasmania and the social issues which impact on service delivery
will also be examined highlighting the limitations imposed upon the
workers. Tasmania has 17 Fairfield Hospital trained GP's who are the
primary care providers with specialist Consultant backup in each of the
three health regions. Their role and ongoing education which enhances
the quality of care provided will also be investigated. HIV services are
now being integrated into the Sexual Health model of service delivery.
The issues which surround this marriage are also explored.
