 AB    GOALS: maximise the general health of clients, minimise the risk of
       transmission of HIV and other diseases. ACTIONS: develop rapport with
       clients & caregivers, assess clients abilities and support network,
       maintain adequate infection control practices, support and liaise with
       members of the clients support network as appropriate. CONCLUSION: Use
       of a periodic assessment form may assist the nurse to identify changes
       in health status and/or clients abilities, increase the standard of
       documented information in the clients files, be an effective teaching
       tool for the nurse.
