 AB    OBJECTIVE: To describe the pattern of diagnosis of AIDS defining
       illnesses among cases of AIDS notified to the National AIDS Registry as
       having been diagnosed during the interval 1990 - 1993. METHODS: Cases of
       AIDS were notified to the National AIDS Registry through State and
       Territory health authorities. Information sought on each AIDS case
       included the date of AIDS diagnosis, CD4+ count and the AIDS defining
       illnesses, separated into definitive or presumptive method of diagnosis.
       RESULTS: Of 2885 cases of AIDS diagnosed in 1990 - 1993, Pneumocystis
       carinii pneumonia (PCP) was the most frequent AIDS defining illness
       (34%). The annual proportion of diagnoses of PCP declined from 38% in
       1990 to 27% in 1993. Overall, the proportion of presumptive diagnoses of
       selected AIDS defining illnesses increased from 25% in 1990 to 41% in
       1993. Presumptive diagnoses of PCP increased from 19% in 1990 to 38% in
       1993. Both presumptive and definitive diagnoses of PCP and oesophageal
       candidiasis had a mean CD4+ count at AIDS diagnosis of less than
       100/microliter throughout the interval 1990 - 1993, whereas both
       presumptive and definitive diagnoses of Kaposi's sarcoma had a CD4+
       count of 100-150/microliter. DISCUSSION: Although the proportion of AIDS
       cases with a presumptive diagnosis of specific AIDS defining illnesses
       increased between 1990 and 1993, immune status of cases with a
       presumptive diagnosis was similar to that of cases with a definitive
       diagnosis.
