 AB    A 24 yr old homosexual man, HIV positive since 1992, presented in 1994
       with a 6 week history of fevers, lethargy and weightloss of 20
       kilograms. His CD 4 count was 30. Apart from a stay in Bangkok in
       1975-6, aged 6 to 7 years, there was no other history of overseas
       travel. His only caving experience was a visit in September 1992 to the
       Mabel cave in the Buchan area, Victoria. Examination revealed a very
       unwell pale man, who was peripherally shut down, hypotensive,
       tachycardic and febrile with a temperature of 40.9 degrees C. He had
       massive hepato-splenomegaly and pancytopaenia. A bonemarrow aspirate
       revealed histiocytes with intra-cellular yeast like organisms and
       subsequently grew Histoplasma capsulatum. He was treated with
       intravenous amphotericin at a dose of 0.7 milligram per kilogram. After
       21 days of treatment with amphotericin and a total dose of 15 milligrams
       per kilogram he was commenced on itraconazole 200 milligrams twice
       daily, with good clinical response. This is the first case of
       disseminated histoplasmosis in an HIV positive patient likely to have
       been acquired in Victoria. A review of the literature on histoplasmosis
       both in Australia and worldwide will be presented.
