 AB    OBJECTIVE: To examine the nature of a novel clinical syndrome of HIV
       related pseudodeafness, caused by delayed speech initiation (DSI) in
       patients with HIV associated dementia complex (HDC). METHODS: In a
       number of HIV+ patients referred for assessment of deafness, the actual
       deficit was found to be DSI caused by HDC in the presence of normal
       hearing. We therefore devised a novel Clinical Speech Production Test
       and validated its speech initiation latencies (SILs) using computer
       analysis of digital sound recordings. RESULTS: In the 15 HDC subjects,
       SILs were significantly greater than those of HIV negative controls, or
       of subjects with cortical dementia (Alzheimer's disease) matched for
       severity of cognitive impairment. CONCLUSION: Delayed initiation of
       speech in HDC may masquerade clinically as deafness and reflects
       subcortical dysfunction.
