 AB    Thrombosis is rare in HIV infection. We report the case of a 57 year old
       male with transfusion acquired HIV infection who died of disseminated
       thromboembolic disease associated with undiagnosed Non-Hogkins Lymphoma
       (NHL) and antiphospholipid antibodies. His CD4 cell count was 500/ul
       (30%). The patient spontaneously developed a painful red, swollen left
       calf. Superficial venous thrombosis of the great saphenous vein was
       confirmed by venous doppler. Aspirin was commenced and the patient
       clinically improved. Within ten days however he represented with
       bilateral deep venous thromboses of the lower limbs. He was admitted and
       anticoagulated with heparin. Despite 'therapeutic' anticoagulation, he
       developed progressive respiratory distress, advancing venous thromboses
       of the lower limbs, digital arterial infarction and an acute abdomen.
       His condition progressively deteriorated and he succumbed twenty four
       days after initial presentation. Autopsy revealed widespread thromboses
       involving aorta, vena cavae and mesenteric vessels. Para-aortic
       lymphadenopathy was secondary to high grade NHL. There was no evidence
       of lymphoma in any of the thromboemboli. Antiphospholipid antibodies
       were positive and his beta 2glycoprotein1 level was low. Initial
       platelet counts and coagulation profiles were normal. Thromboembolism is
       rare in HIV disease. The association of thrombosis, lymphoma,
       antiphospholipid antibodies and beta 2glycoprotein1 in HIV disease will
       be discussed.
