 AB    OBJECTIVE: To investigate the neuropsychological deficits present in
       mild HIV associated dementia complex (HDC). METHOD: Subjects recruited
       from large metropolitan hospital out-patient clinics; all living
       independently in the general community. A comprehensive
       neuropsychological test battery was administered to 3 volunteer samples
       of homosexual/bisexual males; (i) 35 patients who met clinical and
       research criteria for probable mild HDC, (Price Brew rating scale = 1 or
       2) (ii) 27 HIV seronegative (HIV-) controls, (iii) 27 neurologically
       intact AIDS (NI-AIDS) patients, matched with the HDC subjects by CD4
       lymphocyte count for severity of systemic HIV disease. RESULTS: Compared
       with NI-AIDS patients, stage 1 HDC patients performed significantly
       worse in the cognitive domains of executive function, memory and complex
       attention, but not in affect or the cognitive domains of simple motor
       function, orientation, language, or visuospatial construction. Patients
       with stage 2 HDC showed poor performance for the same domains as in
       stage 1 but in addition had impairment in simple motor function and
       language. Within the domains of executive function, memory and complex
       attention, test performance was impaired in a maximum of only one domain
       in all of the HIV(-) controls and 95% of the NI-AIDS subjects. In marked
       contrast, 34 (97%) of the 35 mild ADC patients had abnormal test
       performance in all three of these cognitive domains. CONCLUSION: This
       pattern of neuropsychological deficits in mild ADC is suggestive of a
       subcortical dementia.
