TP 770 - Getting scan lines or "Failed to perform..." error message when playing a MPEG-2 file

SYMPTOM:
When trying to play a MPEG file after some .avi files are played, it fails with the error "Failed to perform Stream Creation with VPM". Another symptom may be scan lines shown at regular intervals while viewing a MPEG-2 file.
CONFIGURATION:
Any ThinkPad 770 running Windows 95 with the DVD Enhanced Video adapter installed trying to play MPEG-2 files.
SOLUTION:
Simply update the display driver for Windows 95 to version 1.01 or later. Click here for the latest driver.
